*The following documents can be either uploaded to the "New Student Enrollment Application Form" OR brought into the main office. Enrollment cannot be completed until these documents are received.
- CURRENT IMMUNIZATION RECORD
- CHILD'S ORIGINAL BIRTH CERTIFICATE
- GUARDIAN'S PICTURE ID
- (2) PROOF OF ADDRESS (Examples of proof of residency can be found at this link)
